We collect personal information about you, the Client, and your Nominated Contact Persons:
a) in the process of facilitating your service enquiry / processing your application
b) when you update your personal details
c) when we communicate with you or your Nominated Contact Persons during the term of your Agreement with us
d) when we communicate with another service provider or assessment agency involved in the coordination of your funding i.e. HCP Case Manager or NDIS Coordinator, My Aged Care Assessor
e) when we communicate with another healthcare provider involved in the management of your care i.e. GP or allied health professional
Please note all calls to and from MePACS are also recorded and stored in connection with your client file for quality assurance and training purposes.

Client information is stored securely within Australia in accordance with all relevant privacy and confidentiality legislation.
Peninsula Health has strict policies and protocols with respect to the collection, use, disclosure and storage of client information.
We have taken measures to ensure that paper records and information stored electronically on our computer systems are secure. Only authorised personnel have access to your information.

The information we collect about you includes:
a) personal information – including date of birth, phone number, email and residential address
b) information related to billing – including funding details for clients with government funding
c) sensitive information – including medical history, health information and other relevant sensitive information you may provide

We collect personal information to enable us to provide the Service, including:
a) installation of the Equipment
b) maintenance or repair of the Equipment
c) contacting your Nominated Contact Persons
d) providing a responsive 24/7 monitoring service
e) tailoring our response in an emergency
f) coordinating ambulance, police and other emergency services when required, and
g) for billing purposes.
4. Disclosure & access to your information
Only authorised personnel have access to your information.
We may provide personal information to our technicians, your Nominated Contact Persons, ambulance, police and other emergency services as required in order to provide our Service.
We may disclose relevant personal information to your Nominated Contact Persons, the ambulance, police and other emergency services as required in an emergency to protect your safety and wellbeing.
You may nominate what information we disclose. However, limiting the information we can provide may impact the quality of the service and care you receive.
Your information is not stored or disclosed outside Australia.
